CHHS Champion Award

Objective: To recognize outstanding employees who have advanced the College of Health and Human Sciences’ mission in ways that significantly impact the environment for student, faculty or staff success. These contributions may be inside or outside the classroom.

Eligibility: Faculty members and professional and scientific staff are eligible. Recipients must be College of Health and Human Sciences employees at the time of the College of Health and Human Sciences Awards Ceremony.

Amount of award: $500 one-time stipend

Number of awards: Two per year (one individual professional and scientific staff award and one individual faculty award), although the College of Health and Human Sciences Awards Committee reserves the right not to select any recipient in a given year.

Criteria: 

Nominees regularly exhibit one or more of these characteristics:

· Collaboration across departments, units, and/or programs to generate positive effects on the college environment where all members of the CHHS community can engage.

· Exemplary performance and positive impact aligned with the college’s strategic plan to cultivate a welcoming and respectful environment where all students, faculty, and staff flourish.

· A continuing commitment to promote programs and initiatives that foster success.

Guidelines:

  1. Complete the nomination form and submit singular nomination packet PDF file via Qualtrics form linked here. Submissions must be in .pdf form and submitted via Qualtrics by 11:59pm on the first Tuesday in February. Please name your PDF file “Award title, Last name of nominee” before submitting it (ex: CHHS Champion Award, Lee).
  2. All letters should be on relevant letterhead, addressed to the College of Health and Human Sciences Faculty and Staff Awards Committee, and signed by letter writer. Nominations for this award may be submitted by an individual or employing unit. Letters should be written specifically for the nomination.
  3. Nominating letter: The nominator must be someone with broad knowledge of the nominee’s efforts and who can speak to the breadth and depth of the nominee’s impact on the College of Health and Human Sciences at Iowa State University.
  4. Curriculum vitae/resume: The nominee’s curriculum vitae should not exceed 25 pages.
  5. Letters of support: Two letters of support from individuals who have either directly benefited from the nominee’s efforts or who have firsthand knowledge of the nominees’ impacts on creating an environment where all members of the CHHS community can engage.
